અથવા ડાઉનલોડ કરેલ ફાઈલ ની ઓપેન કરો અને તેમાં રહેલી બધી ફાઈલો કોપી કરો , અને સ્ટેપ ૦૩ પ્રમાણે અનુસરો..

આધાર ડેમોગ્રાફિ અપડેટ ના ક્રેડીસનલ માટે નીચે આપેલી લીંક પર ક્લિક કરી ફોર્મ ડાઉનલોડ કરી તેમાં જરૂરિયાત મુજબની વિગતો ભરી અમોને મેલ કરવા વિનંતી.સાથે નીચે મુજબના ડોકયુમેન્ટસ ની કોપી મેલ કરવી.
- આધાર કાર્ડ
- પાનકાર્ડ
- આધાર સુપરવાઈઝર / ઓપરેટર ની પરીક્ષાનું પ્રમાણ પત્ર
- સી.એસ.સી નું સર્ટીફીકેટ (જો હોય તો)
- સ્કેન કોપી ઓફ Consent_Letter

Jan Aushadhi Centre - Retail & Distributor : All you need to know about
this scheme – Process, Eligibility, FAQ, Link to letters & The Medicine
List.
By
this document we have tried to answer all the questions/queries related to
opening of Jan Aushadhi centre.
The minimum requirement to open Jan
Aushadhi store is as follows:
·
Minimum
required space of 120 sq. ft.
·
VLEs with
either of the following qualification / VLEs with access to a pharmacist with
the required background Diploma in Pharmacy (D.Pharm.)/ Bachelor
of Pharmacy (B.Pharm)/ Master of Pharmacy (M. Pharm.)
OR
·
VLEs with
access to a pharmacist with the required background
·
Must procure
a Retail drug license in the name of “JAN AUSHADHI” from the concerned State
Drug Authority.
·
A
registration Fees of Rs 2000 to be paid to BPPI (will be deducted from Wallet).
